Insurance  Information

As a courtesy to our patients with dental insurance, we will complete a claim form for surgical services, including estimates for these services. We will process these forms once in accordance with guidelines established the American Dental Association and Academy of Periodontology. If your insurance company requires the completion of additional forms or requests additional information, a fee will be charged for administrative costs. We will assist patients covered by secondary insurance by providing them with the necessary information. However, it is the patient's responsibility to submit and follow up with all secondary insurance coverage.

Please, remember that insurance contracts are made between you and your insurance company. Our contract for services is with you: thus, you are responsible for payment of these services when your insurance carrier does not pay in a prompt and business like manner.
